Lakes won the toss and elected to bat but within no time Old Bar paceman Kieran Green bowled opener Drew Townsend for a golden duck.
Fellow opener Liam Simpson was also dismissed cheaply by Green and at 2/9 the home side looked in trouble.
But cometh the hour, cometh the man and season standout Ben Clark strode to the crease and belted a half-ton, which included four boundaries and a maximum.
Clark quickly ran out of partners and was eventually caught by James Phillips off Dylan Strathdee's bowling.
With the side 7/89, Rhett MacGregor (35) and David Balaam (36 not out) combined to take Great Lakes to a respectable 167.
Green finished with 4/23 which gave Old Bar some confidence for a successful run chase. Strathdee and Tom Kelly also finished with two wickets each.
Old Bar's season was soon brought undone by Lakes' Will Elliott. He annihilated Old Bar's top order, Ryan Williams (12), Declan Attenborough (1) and Patrick Strathdee (1), and finished with 3/9.
The remaining batsmen failed to reached double figures, with the side eventually dismissed for 67 in the 23rd over.
Great Lakes will now face minor premiers Taree United in the decider at Chatham Park on Saturday.
