Police and emergency services were called to the crash about 8.15pm (Sunday, December 4).
On arrival, officers found a Toyota utility and a Volkswagen Golf had crashed head-on.
A man and a woman in the Toyota were treated at the scene and conveyed by NSW Ambulance Paramedics to Port Macquarie Hospital in a stable condition.
A man and a woman in the Volkswagen were airlifted by helicopter to John Hunter Hospital in Newcastle in a serious but stable condition.
Police from the Mid North Coast Crash Investigation Unit are investigating the circumstances surrounding the incident.
Investigators are appealing for any witnesses who have not already spoken to police to come forward and contact Taree Police or Crime Stoppers on 1800 333 000.
