About 4.20pm on Tuesday, June 9, the 86-year-old victim, who moves with the aid of a cane, was walking to her car in the rear car park behind Your Heritage financial planning in Albert Street.
A male described as being young or in his early teens, about 150-160cms tall and wearing new blue jeans, snatched the woman's handbag and ran off in the direction of Taree High School.
The victim was knocked to the ground and injured during the assault.
Inspector Allan Fidock, duty officer at Manning Great Lakes LAC, called the attack "concerning."
"It is disturbing that someone is prepared to do that," said Inspector Fidock.
"Particularly against the elderly or vulnerable in the community, that is concerning."
The 86-year-old victim remains in hospital with a serious shoulder injury.
It is likely she will stay there for several more days.
CCTV has been submitted to the police as investigations continue.
Inspector Fidock said that incidents of bag snatching had been low in the Manning, although they weren't uncommon.
He urged anyone with information to step forward.
"Hopefully other members of the community find this as offensive as we do, and don't play a part in concealing it," he said.
Police believe there may have been several people who could have witnessed the attack.
Anyone with information is urged to make contact with Taree Police on 6552 0399 or Crime Stoppers on 1800 333 000.
